Ready to become part of our corporate fit program… print out the physical activity questioner, application to payroll deduct ( when applicable), and the corporate fit & attendance procedures sheet fill them out make a copy of the application to payroll deduct and give to your HR Dept. You will then bring by the packet to our location. NOTE: You may be required to fill out an information form and, may be charged the pro-rated dues for the month on your first visit.
